If you will be utilizing insurance it is important that you call to authorize services if pre-authorization is required. Unauthorized services will be billed directly to you. Please also obtain the amount of your co-payment/coinsurance, any remaining deductible, and the number of sessions allowed per year.
Please Note: Coverage may exclude specific diagnoses e.g. ADD, ADHD, Autism Spectrum, or a specific service such as group therapy.
Your First Visit
Please bring an insurance card and photo ID to your first visit.
Please also complete paperwork online, fax paperwork, or print and bring the paperwork to your initial appointment.
Payment Policies

All payments should be made via check, debit, or credit card. Cash acceptable for in-person services. Copay/coinsurance must be paid at the time of service.
Using Insurance
Please be advised that having insurance does not guarantee reimbursement. It is your responsibility to confirm your coverage and to ensure that the service being provided is reimbursable under your coverage.
Cancellation Policy
If you do not show up for your scheduled appointment you will be charged a $50 "no call no show" fee. If you provide less than 24 hours advance notice to cancel or reschedule your appointment, you will be
required to pay a $35 missed session fee before your next scheduled
session. If you arrive at a session late, the session will end at it's normal time and you will be charged for the full session. You are responsible for any fees incurred as a result of
missed sessions.

In-Network vs Out-of-Network
If I am an out-of-network provider for your insurance plan, full payment is expected at the time of each visit. I will provide you with a superbill to submit to your insurance company. If a billing statement is needed, it will be sent at the end of the month for you to submit to your insurance provider.
